Sarah K. Reeves, FNP-BC, CDN, is a board-certified Family Nurse Practitioner and Certified Dialysis Nurse with more than 15 years of healthcare experience. Her clinical background includes nephrology, hypertension management, outpatient dialysis, acute inpatient dialysis, and the care of medically complex patients across outpatient, hospital, and intensive care settings.

Sarah earned her Master of Science in Nursing with a Family Nurse Practitioner concentration and holds a Bachelor of Science in Nursing. Her experience as a Certified Dialysis Nurse provides a strong foundation for managing chronic illness, complex medication regimens, transitions of care, and the unique needs of patients living with chronic disease.

Sarah is committed to compassionate, evidence-based care with an emphasis on prevention, patient education, interdisciplinary collaboration, and improved long-term health outcomes. Through Post Acute Clinical Services, she supports coordinated in-facility care delivered 7 days a week by the provider network.
