Adult-Gerontology Primary Care Nurse Practitioner

Linda Lee Meyer, MSN, AGPCNP-BC, is a board-certified Adult-Gerontology Primary Care Nurse Practitioner with extensive experience providing primary, preventive, palliative, and post acute care for adult and geriatric populations. She has dedicated her career to improving the health and quality of life of older adults through comprehensive, patient-centered care across long-term care, rehabilitation, assisted living, hospice, and primary care settings.

Linda has extensive experience caring for residents in skilled nursing and rehabilitation facilities, where she provides comprehensive assessments, manages acute and chronic medical conditions, develops individualized treatment plans, and collaborates closely with nursing staff, physicians, therapists, and facility leadership to promote high-quality care. Her clinical expertise includes the management of heart failure, COPD, chronic kidney disease, hospice and palliative care, medication optimization, preventive medicine, and care transitions. She also performs minor procedures including wound debridement, suture removal, and central venous access device removal.

In addition to her advanced practice experience, Linda has served in nursing leadership roles as an RN Supervisor and Weekend Coordinator within long-term care and rehabilitation facilities. Her leadership background has strengthened her expertise in staff support, clinical operations, regulatory compliance, interdisciplinary collaboration, and maintaining high standards of resident care.

Linda earned her Master of Science in Nursing from Western Connecticut State University, specializing as an Adult-Gerontology Primary Care Nurse Practitioner. She also holds a Bachelor of Science in Nursing from Sacred Heart University and began her nursing career as a Licensed Practical Nurse before advancing through each stage of professional nursing practice. Her decades of clinical experience provide a strong foundation for caring for medically complex adult and geriatric patients.

Through her work with Post Acute Clinical Services, Linda is committed to delivering compassionate, evidence-based care that promotes clinical stability, strengthens continuity of care, and improves quality of life for residents. She supports coordinated in facility care delivered 7 days a week by the provider network.
