Family Nurse Practitioner
Kelli McCray, MSN, APRN, FNP-BC, is a board-certified Family Nurse Practitioner with extensive experience in long term care, telehealth, primary care, infusion therapy, case management, nursing leadership, and clinical education. Her background combines advanced practice care with years of bedside nursing and healthcare leadership, providing her with a broad perspective on the needs of medically complex adult and geriatric patients.
Kelli currently provides advanced practice services through both Signify Health and Theoria Medical. Her experience with Theoria Medical includes telemedicine rounding and on-call coverage for patients in Ohio, Florida, and North Carolina. In these roles, she evaluates patient needs, supports management of acute and chronic medical conditions, provides clinical decision-making remotely, and collaborates with facility staff and other healthcare professionals to promote continuity of care.
Prior to becoming a Nurse Practitioner, Kelli developed extensive experience in infusion therapy and case management, including the care and maintenance of intravenous lines, PICC lines, and implanted ports. She also served in nursing leadership roles including Regional Nurse Manager and Director of Nursing, giving her experience in clinical operations, staff oversight, quality improvement, and the coordination of patient care across teams.
Kelli’s nursing background also includes medical surgical care, home health, geriatrics, acute care, and patient education. In addition to her clinical work, she has served as a nursing instructor in both classroom and clinical settings, teaching subjects including nursing fundamentals, pharmacology, leadership, and transition-to-RN coursework. Her experience as an educator strengthens her ability to communicate complex medical information clearly to patients, families, and healthcare teams.
Kelli earned her Master of Science in Nursing with a Family Nurse Practitioner focus and has pursued doctoral education in nursing practice. She is committed to compassionate, evidence-based care, patient education, interdisciplinary collaboration, and improving access to high-quality healthcare through both in-person and telehealth models.
Through her work with Post Acute Clinical Services, Kelli is committed to delivering patient-centered care that promotes clinical stability, strengthens continuity of care, and improves quality of life for residents. She supports coordinated in facility care delivered 7 days a week by the provider network.
