Paula Price, MSN, FNP-C, is a board-certified Family Nurse Practitioner with more than 20 years of nursing experience and extensive expertise in primary care, behavioral health, home health, hospice, long-term care, medical-surgical nursing, and chronic disease management. She is passionate about delivering compassionate, patient-centered care while promoting prevention, education, and long-term wellness.

Since becoming a Family Nurse Practitioner, Paula has provided comprehensive primary care services in both retail health and private practice settings. As the owner of Haven Total Wellness, PLLC, she evaluates, diagnoses, and treats patients with a wide variety of acute and chronic medical conditions while also providing care for individuals receiving treatment for opioid dependence. Her approach emphasizes individualized care, patient education, and evidence-based treatment plans that support long-term health and recovery.

Prior to advanced practice, Paula developed a broad clinical foundation through years of experience in home health, hospice, behavioral health, long-term care, and acute care hospitals. She has cared for medically complex patients requiring ventilator support, tracheostomy management, enteral feeding, wound care, intravenous therapies, and end-of-life care. Her experience also includes nursing leadership, clinical supervision, staff education, and coordinating interdisciplinary care to improve patient outcomes across multiple healthcare settings.

Paula earned her Post-Master’s Family Nurse Practitioner Certificate from Gardner-Webb University after completing both her Bachelor of Science in Nursing and Master of Science in Nursing Education. Throughout her career, she has remained committed to lifelong learning, compassionate patient advocacy, and providing high-quality care that respects each patient’s individual needs, values, and goals.

Through her work with Post Acute Clinical Services, Paula is committed to delivering evidence-based, compassionate care that promotes clinical stability, strengthens continuity of care, and improves quality of life for residents. She supports coordinated in facility care delivered 7 days a week by the provider network.
