Family Nurse Practitioner
Shakeba S. Weathers is a board-certified Family Nurse Practitioner with extensive experience in primary care, correctional medicine, critical care, emergency medicine, progressive care, infection prevention, and nursing leadership. She is committed to delivering compassionate, evidence-based care while improving patient outcomes through collaboration, education, and preventive healthcare.
As a Family Nurse Practitioner, Shakeba provides comprehensive care for patients with acute and chronic medical conditions, including hypertension, diabetes, asthma, infectious diseases, and behavioral health concerns. Her clinical practice includes comprehensive patient assessments, diagnostic interpretation, medication management, preventive care, and minor office procedures. She works closely with multidisciplinary healthcare teams to coordinate individualized treatment plans while emphasizing patient education, disease prevention, and continuity of care.
Prior to becoming a Nurse Practitioner, Shakeba built a diverse nursing career across intensive care, emergency medicine, intermediate care, medical surgical units, infection prevention, and long-term care. As both a travel nurse and staff nurse, she cared for critically ill patients requiring advanced therapies including mechanical ventilation, continuous renal replacement therapy, vasoactive medications, and complex post-procedural management. She also served in leadership roles as an Assistant Director of Nursing and Infection Control Nurse, where she led quality improvement initiatives, antibiotic stewardship programs, regulatory preparedness, staff development, and infection prevention efforts.
Shakeba earned her Master of Science in Nursing as a Family Nurse Practitioner from South University after completing her Bachelor of Science in Nursing at Chamberlain College of Nursing. Her background in both bedside nursing and healthcare leadership provides a strong foundation for delivering safe, coordinated, and patient-centered care across a wide range of clinical settings.
Through her work with Post Acute Clinical Services, Shakeba is committed to providing compassionate, evidence-based care that promotes clinical stability, strengthens continuity of care, and improves quality of life for residents. She supports coordinated in facility care delivered 7 days a week by the provider network.
