Family Nurse Practitioner
Joseph Kiswii, DNP, MPH, MBChB, FNP-BC, is a board-certified Family Nurse Practitioner with a unique background that combines advanced nursing practice, public health leadership, nephrology, geriatrics, and prior physician training. He brings extensive experience caring for medically complex older adults and patients with chronic illnesses across hospital, post-acute, long-term care, home-based, and outpatient settings.
Joseph is currently completing a Geriatrics and Extended Care Nurse Practitioner Residency at the Durham VA Health Care System, where he provides comprehensive care for older adults in community living centers, home-based primary care, hospice and palliative care, geriatric primary care, and specialty clinics. His practice includes comprehensive geriatric assessments, medication management, cognitive and functional evaluations, falls prevention, goals-of-care discussions, and interdisciplinary care coordination for vulnerable older adults.
Before becoming a Family Nurse Practitioner, Joseph built more than a decade of experience in nephrology and dialysis care, serving in leadership, quality improvement, acute dialysis, and clinical management roles with organizations including DaVita Kidney Care, Fresenius Medical Care, and the IPRO ESRD Network Program. He led initiatives to improve care transitions, strengthen communication between dialysis providers and skilled nursing facilities, reduce avoidable hospitalizations, and enhance quality outcomes for patients receiving hemodialysis. His experience also includes regulatory compliance, CMS quality initiatives, and interdisciplinary collaboration across healthcare systems.
Joseph earned his Doctor of Nursing Practice from the University of North Carolina Wilmington, where his doctoral project focused on implementing health literacy training and assessment within an outpatient dialysis clinic. He also holds a Master of Public Health from the University of California, Berkeley, and completed medical training as a physician, earning a Bachelor of Medicine and Surgery before pursuing advanced nursing practice in the United States. This diverse educational background provides him with a broad clinical perspective and a strong commitment to evidence-based, patient-centered care.
Through his work with Post Acute Clinical Services, Joseph is committed to delivering compassionate, evidence-based care that promotes clinical stability, supports treatment in place whenever appropriate, strengthens continuity of care, and improves quality of life for residents. He supports coordinated in facility care delivered 7 days a week by the provider network.
