Shavonne Jackson, MBA, MSN, APRN, FNP-C, CNL, is a board-certified Family Nurse Practitioner and Clinical Nurse Leader with experience in primary care, geriatric medicine, chronic disease management, nephrology, dialysis care, and clinical leadership. Her diverse background supports a comprehensive approach to caring for medically complex patients while promoting continuity, collaboration, and improved health outcomes.

Shavonne has provided primary care for geriatric and underserved populations, managing acute and chronic conditions through comprehensive assessments, diagnostic evaluation, medication management, patient education, and individualized treatment planning. She has also provided care in correctional healthcare settings, where she evaluated and treated patients with a broad range of healthcare needs while supporting continuity of care.

Her extensive nephrology experience includes managing patients with chronic kidney disease and end-stage renal disease in both inpatient and outpatient settings. She has conducted rounds across multiple dialysis centers, managed dialysis treatments and medications, participated in comprehensive assessments and quality improvement initiatives, and provided education and support to patients and their families. Her previous leadership roles in dialysis care strengthened her experience in clinical operations, regulatory compliance, staff development, quality improvement, and interdisciplinary collaboration.

Shavonne earned her Post-Master’s Certificate as a Family Nurse Practitioner from South University and her Master of Science in Nursing as a Clinical Nurse Leader from the University of Virginia. She also holds a Master of Business Administration from Liberty University and a Bachelor of Arts in Mass Communications from Virginia Union University. Her combination of advanced clinical education, healthcare leadership, and business experience provides a well-rounded perspective on patient care and clinical operations.

Through Post Acute Clinical Services, Shavonne is committed to delivering compassionate, evidence-based care that supports clinical stability, strengthens care coordination, and improves outcomes for residents in skilled nursing and post-acute care settings. She supports in-facility care 7 days a week through the provider network.
