Nicole Hopper is a board-certified Family Nurse Practitioner with experience in long term care, skilled nursing, primary care, medical surgical nursing, dialysis, chronic disease management, and clinical education. Her diverse healthcare background supports a comprehensive, patient centered approach to caring for adult and geriatric populations with complex medical needs.

Nicole currently provides advanced clinical care within long term care settings, where she focuses on comprehensive patient assessment, chronic condition management, medication management, care coordination, and continuity of treatment. Her experience in skilled nursing and post acute environments provides her with a strong understanding of the clinical needs of medically complex residents and the importance of close collaboration with facility staff, physicians, specialists, patients, and families.

Prior to becoming a Nurse Practitioner, Nicole developed extensive nursing experience in inpatient dialysis, medical surgical care, short stay and observation medicine, and correctional healthcare. She spent nearly eight years providing acute dialysis care, managing complex treatments, evaluating laboratory findings, administering medications, and supporting patients with advanced kidney disease. She also served as an RN Unit Coordinator, leading nursing staff while promoting communication and coordinated care across interdisciplinary healthcare teams.

Nicole earned her advanced Family Nurse Practitioner education from Chamberlain University and holds a Bachelor of Science in Nursing from Sentara College of Health Sciences. In addition to her clinical work, she has served as adjunct nursing faculty, providing evidence based clinical education, hands-on instruction, and mentorship to future nurses.

Through her work with Post Acute Clinical Services, Nicole is committed to delivering compassionate, evidence based care that promotes clinical stability, strengthens continuity, and improves quality of life for residents.
