Letisha Driskell is a board certified Family Nurse Practitioner with clinical experience caring for medically complex adult and geriatric patients in post acute and long term care settings. She focuses on improving patient outcomes through thoughtful chronic disease management, medication optimization, and collaborative care planning.

Letisha currently provides care in a long term acute care hospital where she performs comprehensive patient evaluations, conducts daily rounding, and manages complex conditions such as heart failure, COPD, diabetes, renal disease, and infectious illnesses. She works closely with interdisciplinary care teams including nursing, therapy, case management, and social work to coordinate treatment plans and support safe transitions of care.

Her clinical approach emphasizes compassionate patient centered care, proactive monitoring of chronic conditions, and strategies that help reduce avoidable hospital readmissions. She also has experience guiding goals of care discussions with patients and families while supporting palliative and supportive care planning when appropriate.

In addition to her post acute clinical work, Letisha has experience managing metabolic and chronic health conditions in outpatient settings. She integrates patient education, lifestyle counseling, and evidence based medical treatment to support long term health improvement.

Letisha earned her Master of Science in Nursing as a Family Nurse Practitioner and is currently pursuing a Psychiatric Mental Health Nurse Practitioner certificate at Liberty University. She holds board certification as a Family Nurse Practitioner and maintains active RN licensure in Virginia along with Advanced Cardiac Life Support and Basic Life Support certifications.
