Faith Lynn Pletcher is a board certified Family Nurse Practitioner with extensive experience in post acute care, skilled nursing medicine, orthopedic and trauma nursing, and chronic disease management. She is dedicated to delivering compassionate, patient centered treatment focused on improving outcomes and enhancing quality of life for medically complex patient populations.
Faith currently provides primary care services within skilled nursing and post acute care facilities through Theoria Medical, where she manages both acute and chronic medical conditions including diabetes, hypertension, infections, injuries, and complex multisystem illnesses. Her clinical expertise includes diagnostic interpretation, medication management, preventive care, and interdisciplinary coordination designed to support continuity of care and reduce avoidable hospitalizations.
Prior to becoming a Nurse Practitioner, Faith built a strong nursing foundation through years of experience caring for orthopedic, surgical, trauma, and medical surgical patients across multiple healthcare systems. Her background as both a staff nurse and travel nurse provided broad exposure to diverse clinical settings while strengthening her leadership, communication, and patient advocacy skills.
Faith earned her Master of Science in Nursing with a specialization as a Family Nurse Practitioner from South University and her Bachelor of Science in Nursing from Methodist University. She maintains board certification through the ANCC and also holds wound care certification through Vohra Wound Physicians.
Through her work in post acute medicine, Faith is committed to delivering reliable, evidence based care that supports in facility treatment and ensures coverage 7 days a week through the provider network.
