Trinette McLain is an Adult Nurse Practitioner with extensive experience caring for adult and geriatric patients across post acute, primary care, hospice, and long term care settings. She specializes in managing chronic disease conditions, coordinating interdisciplinary care, and supporting older adults with complex healthcare needs.

Throughout her career, Trinette has worked in skilled nursing facilities, senior clinics, and medical management programs, where she has focused on reducing unnecessary hospital transfers and supporting patients through individualized care plans. Her clinical experience includes comprehensive patient assessments, interpretation of diagnostic testing, medication management, preventive care, and education for both patients and caregivers. She has also collaborated closely with physicians, hospice teams, social workers, and care coordinators to ensure continuity and quality of care across settings.

Trinette’s background includes advanced practice roles with Optum Health, Geriatric Consultants, Healtheast Medical Care for Seniors, Matrix Medical Network, and Park Nicollet Clinic. Her work has consistently emphasized patient centered care, chronic disease management, care coordination, and health promotion for vulnerable adult and geriatric populations.

She earned both her Bachelor and Master of Science in Nursing from Metropolitan State University and maintains licensure in both North Carolina and Minnesota.
